Record Search Request Form

This form is designed to guide the applicant step by step, clearly explaining what information is needed, why it is requested, and how it will be used during the search process for a Dominican civil record. Each step includes instructions and examples to avoid confusion and ensure the search is as accurate and fast as possible.

Step 1 of 4

Applicant Information

This is where you’ll receive confirmations, clarification requests, and preliminary results

Service Fee
USD $95 for the record search.
If the record is from before 1945, the correct fee is USD $195.
You will only pay the difference once we notify you.
Even so, the search begins with your $95 payment.
If you don’t know the location where the record was created, a fee of USD $195 also applies, as we would need to search in over 160 offices nationwide.
Again, you would only pay the difference if applicable.

Start of the Search
The search only begins once the payment is confirmed.
As soon as PayPal confirms your payment, we start immediately.

After Payment: What Happens?

  • Request Confirmation: You will receive a confirmation email indicating that we have successfully received your information.
  • Result of This Stage: This is the first phase of the process.
    The result is the Record Search Sheet, which we will send to your email.
  • Data Review: You should review the Record Search Sheet and confirm that all the record information is correct.
  • If Something is Incorrect: Report it, and we will handle the correction for you.
